Property and account fields
The address, owner or customer account and municipal identifiers attach the result to the correct record. A correct test with a wrong account number can create an administrative mismatch.
Compare these fields with the current notice. If the property has multiple buildings or units, confirm that the report describes the tested service accurately.
Assembly identification
Look for manufacturer, model, size, serial number and assembly type. These should describe the installed device rather than a generic product category.
Replacement changes the serial number and sometimes the model. The report should make that change visible so the municipality can update its inventory.
Location and service protected
A location such as mechanical room is often too vague on a large site. The best reports identify the room, wall, elevation or nearby equipment and state whether the assembly protects irrigation, domestic, fire or another service.
That detail supports future access, emergency response and portfolio reconciliation. It also helps prevent a different device from being tested by mistake.
Readings and final result
The numerical fields depend on whether the assembly is an RPZ, DCVA, PVB or another accepted type. The tester compares those readings with the applicable performance criteria and records pass or fail.
If work is performed, distinguish the initial failed readings from the final post-repair readings. A repair note without a passing retest does not close the testing cycle.
Tester and equipment records
Reports commonly include the tester's name, certification number, company, signature and test-kit identification or calibration date. Municipalities use these fields to validate the person and equipment behind the result.
Finally, retain evidence that the report reached the accepted City system. A signed paper in the property file may not be enough where the program requires a portal submission or fee.
